H1N1 VACCINATIONS

WHO:   High School and Middle School Students

WHEN:  Friday, December 11, 2009

WHERE: Nutley High School - 9:30 to 11:30 a.m.
        John H. Walker Middle School - 11:30 to 1:30 p.m.

The Township of Nutley's Department of Health and Public Affairs will  
offer free inoculations to all high school and middle school students.

THESE INOCULATIONS ARE VOLUNTARY AND NOT A MANDATED REQUIREMENT

Students must bring a complete and signed permission form with them on  
the day the inoculations are given.  These permission forms will be  
backpacked home and are also available at NUTLEYNJ.ORG.

Only students with a complete and signed permission form will be inoculated.

The inoculations will be administered by Township Health Department personnel.
